I am participating in the Pilates, exercise classes offered by Prehab Pilates LLC in which I will receive information and instruction about Pilates, fitness and health. I recognize that these classes require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the classes, Health programs or workshops. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the exercise Classes, health programs or workshops.
In consideration of being permitted to participate in exercise Classes, health programs or workshops, I agree to assume full responsibility for any risks, injuries or damages known or unknown, which I might incur as a result of participating in the program.
In further consideration of being permitted to participate in exercise classes, , Health programs or workshops, I knowingly, voluntarily and expressly waive any claim i may have against Prehab Pilates LLC for injury or damages that I may sustain as a result of participating in the program.
Our nutritional counseling program, and guide cannot and should not be used in lieu of medical care for your illness. Prehab Pilates and Magdolna Faucera does not practice medicine, our program is designed to focus on nutrition and lifestyle habits to optimize health while you receive medical care from your current physician. We recommend you consult your doctor before doing any nutrition program.
I hereby authorize Prehab Pilates to publish photographs taken of me, and my name and likeness for the use of Prehab Pilates’s print, online and video marketing materials, including other company publications.
I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue Prehab Pilates LLC for any injury or death caused by their neglegence or other acts.
